requested of you and if your driving privileges have been previously suspended for a prior refusal to submit to a lawful test of your breath urine and blood you'll be committing a Mr meter refusal to submit to this test I request that you as admissible into evidence in any criminal proceeding additionally if you hold a commercial driver's license or you're operating a commercial motor vehicle your commercial driver's license PIV also be disqualified for one year for the first offense or permanently disqualified if you've been previously disqualified under Section 32264 Florida statute okay so
