The incident occurred just before 3 p.m. near Washington High School. Officers say they were investigating a domestic aggravated assault from earlier this month. Police say they saw a man drive past a victim's home multiple times. Officers were then called to locate the man. The man matching the description jumped a fence into a nearby parking lot, according to police. After officers attempted to contact the man, they say he allegedly fired a shot from a handgun. One officer then fired his gun, according to police. The man later died from his injuries. He has been identified by police as 41-year-old Derin Holmes. In a critical incident report released two weeks after the shooting, officials say Holmes died from a self-inflicted gunshot wound and the shots fired by officers did not strike him. Phoenix police say no officers were injured.
