On Thursday, November 13, 2025, the Los Angeles Police Department authorized the release of the names of the officers involved in an Officer-Involved Shooting (OIS) that occurred on Tuesday, November 11, 2025, in the Southeast Division. The officers involved were identified as: Police Officer II Agustin Hernandez, Serial No. 37634 Police Officer III Jay Weber, Serial No. 43266 According to preliminary information, at approximately 11:50 a.m., Southeast Patrol Division officers responded to a call of an assault with a deadly weapon near a residence in the 10300 block of Kalmia Street. Call comments indicated the suspect, later identified as 56-year-old Hugo Polanco, was allegedly using metal wires as a whip and attempting to strike passersby. Upon arrival, officers observed Polanco holding a cluster of metal wires approximately 7.5 feet long. Officers ordered him to drop the weapon. Although he initially complied, Polanco picked the weapon back up. Officers deployed a 40mm less-lethal launcher, which proved ineffective. Polanco then allegedly charged toward an officer, gripping the wires with both hands and raising them in a whipping motion. An Officer-Involved Shooting occurred. Polanco was struck by gunfire, taken into custody, and transported by Los Angeles Fire Department personnel to a local hospital, where he was later pronounced deceased. No officers or community members were injured during the incident. The weapon was recovered and booked as evidence. Notifications to the next of kin were conducted by LAPD’s Family Liaison Section.
